Through RIQI’s Transforming Clinical Practice Initiative (TCPI), Brown Surgical Associates were recognized by the Center for Medicare and Medicaid Services (CMS) as an exemplary practice with an entire team committed to quality improvement through coaching, peer-to-peer learning, and the use of innovative quality improvement tools. At Brown Surgical Associates, we want you to know that we’re continually striving to improve patient outcomes. Our strongest quality initiative is our Post Discharge to Home Phone Call program that focuses on assessing for a safe transition to home to avoid hospital readmissions, assess for the early development of any potential complications and to provide educational support. As a patient, if you are discharged to home after a hospital stay please know that one of our team members will be calling you to ensure you are doing well, answer your questions and even schedule an appointment to be seen if it is determined to be necessary.
